The investigation blames human error. If the Russian pilots had of followed the TCAS instruction to climb instead of the ATC instruction to descend, the crash would never have happened. Additionally if better procedures were in place to inform the ATC's of their vital equipment being dowin during maintenance, would not have heppened either.
